Legal Disclaimers and Disclosures
Membership clubs are not insurance but a payment agreement provided by Oyster Dental Clinic LLC for services rendered. Memberships are provided exclusively to patients without insurance at our clinic and should not be considered prepayment for future services or payment for discounted access to services. Instead, our member patients are choosing to make regular monthly subscription payments for preventive care, rather than paying at the time of each service. Payment for any cosmetic or restorative treatment must be made at the time of service. Membership fees must be current to receive services. A lapse of 28 days in annual dues will require a six-month reactivation fee for dues. Membership club discounts cannot be combined with other offers or discounts. It is the sole responsibility of the patient to schedule and maintain their appointments. No refunds will be provided for fees paid under any circumstances, including for missed appointments or failure to maintain scheduled visits. The monthly contract of a member patient is with the legal entity of Oyster Dental Clinic LLC. Monthly dues and dental service fees may change at any time. Payment management services are provided by illumitrac℠.

Just schedule an appointment at our office and make sure to let us know you’re a member. When you arrive, we’ll verify that your membership is active, and you’ll be eligible for instant savings on our regular fees.
If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.
It will be your responsibility to reschedule. Please refer to our clinic policy regarding no-shows and last-minute cancellations.
Absolutely! Your entire family can join.
Your club membership is exclusive to our clinic and, as such, is non-transferable. If you move to another area, you can cancel your membership at any time.

Yes! Just log in to the Member Portal and click on “Edit Your Payment Method.”
Log in to the Member Portal and click the “Remove” button for each member you wish to cancel.
No. There will be no refund of fees paid under any circumstances, including for missed appointments or failure to maintain scheduled visits.
You will only pay the Lifetime Activation Fee to join. This fee includes the membership fees for the first month.
